Hourly Rate/Salary: $125k - $188k


Job Summary:


A leading healthcare organization in New York is seeking a Manager, Case Management (RN) to oversee and ensure high-quality case management services within an acute care setting. This is an excellent opportunity for an experienced leader to contribute to patient-centered care while optimizing operational efficiency.


  • Overseeing case management operations to ensure compliance with hospital policies and regulatory standards.

  • Managing department budgets, staffing, and workflow to maintain efficient patient care and timely discharges.

  • Implementing performance improvement initiatives and tracking key quality metrics.

  • Providing leadership and professional development opportunities for case management staff.

  • Collaborating with interdisciplinary teams to enhance care coordination and patient outcomes.

  • Ensuring compliance with Medicare, Medicaid, and private insurance regulations.

  • Maintaining strong relationships with community agencies to facilitate seamless patient transitions.

Requirements

  • Active NYS RN license required, Bachelor’s Degree in Nursing (BSN) required, Master’s Degree preferred, PRI/Screen certification, CCM certification preferred

  • 5+ years of managerial experience in an acute care hospital setting.

  • Previous experience as a case manager in acute care setting required.

  • Proficiency in Milliman/InterQual criteria and electronic medical record systems.
